From medical emergencies to alleged assassination attempts, several key witnesses have failed to appear before the inquiry, causing repeated delays

The Madlanga Commission of Inquiry has repeatedly faced delays after several key witnesses failed to appear for their scheduled testimonies.
Many of the postponements have been attributed to illness, medical emergencies, or other last-minute reasons.
The Commission’s chairperson, Chief Justice Mbuyiseli Madlanga, has criticised the repeated use of vague medical certificates.
“I notice that this is one of those medical certificates, the nature of which I’ve previously said is useless, in the sense that they give no information whatsoever. It’s not even a matter of confidentiality. It’s just one of those useless medical certificates,” Madlanga said on Monday, 13 July 2026, after IDAC head Advocate Andrea Johnson failed to appear.
